Lost deep in the dark forest, a small unnoticed patch of mud trips the party. As they scramble to their feet, roots become bones. Bones below become bones above as each character finds themselves stuck in the mud with a grotesque creature scrambling atop them.

The creatures are uncanny doppelgangers, who seem to be trying to escape the mud.

A fearful desperate grapple ends with the double fleeing into the woods.

Each character takes d6 damage and is stunned. After a few moments they find themselves simply laying in mud, but will forever be haunted by the idea that another of them exists out there somewhere in the forest.
